We currently have Flutes & Clarinets ready to go.
You can complete your rental from the comfort of your own home, you just need a PayPal account & around 5 minutes just follow the following instructions:
Type information into the form.
Choose Clarinet or Flute.
Choose to pay by the month or 3 months at a time.
Leave Make/Model/Serial # blank, I will fill that in.
Print the completed form (or If you have a digital signature feel free to use it)
Sign & Date the bottom.
Either take a picture or scan the form to produce a digital copy (Jpeg or PDF are preferred)
Take a clear picture of the BACK of your driver’s license.
This is for me to verify you are who you say you are.
(I do not require your Social Security number)
Email form AND Picture of License to : chad@baileysbandroom.com
Once we receive the contract, we will send you an invoice via your paypal acct.
Please pay it at that time and I will
fill out your tags & deliver the instrument to your school within 3 School Days.
